Abstract

The purpose of this study was to evaluate a bottom-up implementation of three regional disease management programs on chronic obstructive pulmonary disease (COPD) based on bottlenecks experienced in professional practice. We performed a multi-site comparison of three Dutch regional disease management programs combining patient-related, professional-directed and organizational interventions. Process (Assessing Chronic Illness Care (ACIC) survey) and outcome (disease specific quality of life (clinical COPD questionnaire, CCQ; chronic respiratory questionnaire, CRQ), MRC dyspnoea, and patients’ experiences) data were collected of 370 COPD patients and their care providers.
